Course Outside of Program Approval Form
Directions: Use this form to identify coursework outside of Department offerings that you would like approved as credit toward the M.A. Degree in Rehabilitation & Mental Health Counseling. Transfer credits must be from a program accredited by the Council on Rehabilitation Education (CORE) or the Council for Accreditation of Counseling and Related Educational Programs (CACREP). Courses that were completed to meet the requirements for another degree cannot be counted as transfer credit toward a second degree. Transferred courses must be at the graduate level and acceptable to the program. A syllabus must be attached for each outside course for which approval is requested. A grade of B or better must be earned in order for transfer credits to be approved. Field placement coursework will not be accepted as transfer credits under any circumstances. Courses that were earned seven or more years before the student applied for admission to the program will not be accepted as transfer credit. You must submit this request to the Department Chair no later than four (4) weeks prior to the start of the semester during which you expect to enroll for the course. The Chair can approve the request, with a copy to the student's Advisor and the Instructor of Record; or she/he can forward it to the Instructor of Record and the student's Advisor for their further consideration and approvals. Once the request is approved you must complete a Graduate Course Transfer Form to have the course officially transferred into the program.
